According to a CBS news report, an investigation of the FDA suggests serious deficiencies. Investigator George Nedder and the Office of Inspector General reviewed 30 voluntary food recalls from 2012 to 2015 and found potential critical failures.
Nedder said, "23 of the 30 recalls...were Class I. By definition, they can cause a serious, irreversible medical condition...or potentially can cause death." Regarding those recalls, the report says the FDA could not always ensure that firms initiated the recalls promptly.
